On July 7th, 2013 an update will be posted to all monthly SpEd Forms sites. This update will include the changes detailed below:


  1. Wording changes were made to the Annual and Initial IEP Denied letters.

Evaluation Reports

  1. Added Assistive Technology to the Evaluation Area drop down list.

  2. The Deaf/Hard of Hearing criteria and the eligibility determination section of the evaluation report template have been updated.

  3. The Physically Impaired criteria has been updated.

  4. Updated the School Function Assessment (SFA), Developmental Test of Visual-Motor Integration –
    6th Edition (VMI), Sensory Profile School Companion, Sensory Profile School Companion (expanded), and Sensory Processing Measure (SPM) tests.

  5. Two new tests were added, Social Language Development Test – Elementary (SLDT-E) and Social Language Development Test – Adolescent (SLDT-A).

Individualized Education Program

  1. On the student setup, an Exit date field has been added.
