WI March Updates – SpEd Forms

On Saturday, March 1st, 2025, an update¹ will be posted to all Wisconsin SpEd Forms sites. The update will include the changes listed below:

IEP

1. When an annual IEP is amended and finalized into History, the P-2 placement form will pull from the Amended date.

1. Formatting changes were made to printed and finalized 504 plans in section 4: team placements.

Reports

1. The Admin report “DPI SpEd records” will now be restricted to suggested records for a specific school year.

1. P-6 Notice of Cessation of Special Education and Related Services in Response to Parental Revocation of Consent, P-4 Notice of Ending Services Due to Age and I-11 Extended School Year have all been converted to v2.
